FOR A RETURN AUTHORIZATION NUMBER. ALL CUSTOMER RETURNS MUST BE AUTHORIZED PRIOR TO RETURN. THE RETURN AUTHORIZATION NUMBER SHOULD BE PLAINLY MARKED ON THE OUTSIDE OF THE BOX. INCLUDE A COPY OF THE INVOICE OR PACKING LIST WITH YOUR RETURN.

RETURNED ITEMS SHOULD BE SENT TO 2361 SOUTH 1560 WEST, WOOD CROSS, UT 84087.

ALL ITEMS ORDERED BY YOU IN ERROR WILL BE CHARGED A TWENTY-FIVE(25) PERCENT RESTOCKING FEE.
WE DO NOT ACCEPT MARKED. ALTERED, OR MODIFIED PRODUCTS FOR CREDIT OR EXCHANGE.

ANY CLAIMS FOR DISCREPANCIES IN SHIPMENT MUST BE MADE WITHIN SEVEN (7) DAYS OF RECEIPT. FAILURE TO INSPECT ITEMS WITHIN SEVEN DAYS SHALL CONSTITUTE FULL ACCEPTANCE OF ORDERS.

ALL ITEMS SHOULD BE INSPECTED IMMEDIATELY UPON RECEIPT AND ANY DAMAGE WHICH OCCURRED IN SHIPMENT SHOULD BE REFERRED TO THE CARRIER.

A FINANCE CHARGE OF 1 ½ PERCENT PER MONTH OR 18 PERCENT INTEREST PER ANNUM WILL BE ASSESSED AGAINST ANY DELINQUENT ACCOUNT.
